Licensed Practical Nurse - Primary Care Clinic - Full Time

Primary Care NurseUnder the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.Mercy Clinic Internal Medicine - Medical Tower A Suite 189Location: 621 S. New Ballas Road, Medical Tower A, Suite 189-A, St. Monday – Friday, 0800-1630No nights, weekends, or holidays!Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic.
