Employment Type: Travel Contract (Home Health LPN / Travel Nurse)
Shift: Monday - Friday, 8:00 AM - 5:00 PM (Day Shift)
Shift Duration: 8-hour days
Contract Length: 13 weeks (travel LPN assignment)
Mileage Reimbursement: $0.43 per mile
Estimated Weekly Pay: $1460-$1700/week
Active LPN license in Indiana or compact state (Licensed Practical Nurse)
Minimum 1 year of home health experience required (homecare / home health nurse)
Proficiency with HomeCare HomeBase (HCHB) software
Ability to use Microsoft Teams and Outlook for interdisciplinary communication
Provide high-quality home health nursing care to patients in their homes across Carmel and the Greater Indianapolis area
Perform patient assessments, administer medications, and monitor patient progress during homecare visits
Document care accurately using HCHB (HomeCare HomeBase) and communicate updates through Teams and Outlook
Collaborate with interdisciplinary healthcare teams to coordinate care plans and ensure optimal patient outcomes
Educate and support patients and family caregivers on care plans, medication administration, and self-care
