Home Health Physical Therapist (PT)We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in Anderson, MO and surrounding area. Provide clinical care where patients want to be – in the comfort of their own home. $15k Retention Bonus Available for Full Time onlyWhy you'll love what you do at Integrity Home Care + Hospice:Experience true work/life balance with flexible full-time, part-time, and per diem schedulesPerform 1:1 direct care in client homesHands-on training supplemented with virtual learning24/7 clinical support as part of a larger interdisciplinary care teamAdvance your career with clinical and business leadership tracksWhat you'll do:Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.Responsibilities for a PT - Physical Therapist include:Make home visits to clients in designated geographic territories.Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.What you should have:Active and current PT licenseOne year of clinical experience preferredPrevious home health care experience welcomed, but not required.
