By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.Affordable housing (actually own a home and not rent)Lakes, trails, and all four seasons- without big-city burnoutFamily Friendly Communities & Short commutesTight-knit teams where you're more than a numberAs a Home Health Physical Therapist with CenterWell Home Health you will:Receive a Sign-On BonusPlan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.Document patient observations, interventions, and evaluations promptly and thoroughly.Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.Required Qualifications:Degree from an accredited Physical Therapy Program (approved by the APTA)Current and unrestricted Physical Therapy licenseCurrent CPR certificationExperience collaborating with a team of healthcare professionalsPreferred Qualifications:One year of experience as a Physical TherapistPay Range$59.00 - $83.00 pay per visit/unit$93,000 - $128,000 per year base payAdditional InformationThis role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: wireless, wired cable or DSL connection is suggested.
