CareNational is seeking a skilled and compassionate Physical Therapist (PT) for a full-time contract with a well-regarded Long-Term Care/Post-Acute Rehab facility in Medford, OR . This is an excellent opportunity for a licensed therapist who enjoys helping residents regain mobility and independence while working a consistent weekday schedule.

Onsite | Rehab Department - Long-Term Care / Post-Acute Facility

Schedule: Monday through Friday

Shift: Variable Shift

Type: Full-Time Contract

Start Date: ASAP

Physical Therapist License - Oregon (Required)

Licensed to practice as a Physical Therapist in Oregon

Prior experience in long-term care, rehab, or post-acute settings preferred

Evaluate residents to determine therapeutic needs and potential

Provide direct therapy services in accordance with physician-approved plans

Document evaluations, treatments, progress notes, and charges accurately

Participate in discharge planning to support ongoing patient success

Supervise Physical Therapist Assistants and Aides

Screen residents for potential benefit from physical therapy interventions

Ensure compliance with facility, state, and federal standards
