Hospice Care Center RN
Gentiva Hospice
Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings. You will perform comprehensive assessments, develop care plans, and coordinate with an interdisciplinary team to deliver personalized end-of-life care.
Responsibilities include teaching caregivers, documenting care, and ensuring quality, compliant nursing services while supporting patients and families through the hospice journey.
