LPN - Part Time Primary Care

Job Description Franklin Township Partnership Health Center is seeking a compassionate, organized, and patient-focused Licensed Practical Nurse (LPN) to join our healthcare team on a part-time basis (25 hours per week) . This role is ideal for an experienced LPN who enjoys working in a collaborative primary care environment and is committed to providing exceptional patient care.

The LPN will support physicians and clinical staff by providing direct patient care, coordinating follow-up services, managing patient documentation, and assisting with chronic disease management programs. The successful candidate will demonstrate excellent clinical, communication, and organizational skills while maintaining a high standard of patient-centered care.

Responsibilities include:
Obtaining full history on all patients, including medications, medical history, surgical history, social history and depression screening
Assisting physicians with exams
Ordering labs, testing and medications
Holter monitor application
Blood pressure monitoring
Chronic care management
Schedule - 2 (two) - 10 hour days and every other weekend (9a-1p)
Qualifications
Current New Jersey Licensed Practical Nurse (LPN) license in good standing
Graduate of an accredited Practical Nursing program
Prior experience in a primary care, family medicine, internal medicine, or outpatient setting preferred
Strong knowledge of medical terminology and electronic medical record (EMR) systems
Excellent communication, interpersonal, and organizational skills
Ability to work independently and as part of a multidisciplinary healthcare team
Proficiency in patient assessment and clinical documentation
