Position: Occupational Therapist (OT) - Home Health Employment Status: Per-Diem, Part-Time Location: Work in the area you prefer—your caseload follows your location choice.

Essential Job Functions and Responsibilities:

Evaluate patient's functional status and participates in development of plan of care

Develop treatment program and establish goals for improved function

May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury

May train patient in the use of prosthetic and/or orthotic device

Maintains good communication with other personnel involves in the patients care

Qualifications and Skills:

• Occupational Therapist with current licensure
• Minimum of two (2) year experience.
• Self directed and able to work with minimal supervision.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
