Brockton Home Health Care AFC is looking for A registeredNurse (RN)/Case Manager to manage and support our Adult Family/Foster to makesure the our programs runs smoothly.
Job Title: MDS Nurse Assessment Coordinator for AFC in Brockton
Compensation: $100 per MDS Nurse Assessment and $50 per visit
Employment Type: Full-Time or Part-Time
We are seeking a caring and detail-oriented Registered Nurse to join our AdultFoster Care (AFC) program as an Initial Assessment / Admission Nurse. This opportunity isideal for nurses with experience in geriatrics, home health, or community-basedcare who are committed to supporting individuals and enhancing quality of life.
Evaluate medical, functional, and psychosocial conditions to determine eligibility.
Partner with caregivers, families, and healthcare professionals to design customized care plans.
Provide education to participants and families regarding AFC services, expectations, and program policies.
Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.
Demonstratedability to coach, train and provide staff development
- Proficiencyin Microsoft (Word, Excel and PowerPoint)
- MS or Bachelor'sdegree in Nursing
- Must befully licensed RN by Mass Board of Registration
- At leastfive years of previous home health care experience
