Family Medicine Physician Hybrid Primary Care (Home Visits + Clinic)

Community-based primary care practice in Brooklyn is seeking a full-time Family Medicine Physician to provide a hybrid model of outpatient care in Bedford-Stuyvesant, combining in-office visits with scheduled home care.

This opportunity is ideal for physicians who value continuity of care and hands-on community medicine.

Position Overview

The physician will provide comprehensive primary care to adult patients in a hybrid format, typically split between 23 days of home visits and 23 days in-office. Responsibilities include preventative care, chronic disease management, wellness visits, and ongoing patient follow-up.

This is a fully outpatient, permanent role supported by an established patient base and experienced administrative staff.

Qualifications

MD or DO

Board Certified or Board Eligible in Family Medicine

Active New York license (or eligibility)

Outpatient primary care experience preferred

Comfortable with hybrid care model

EMR proficiency
