Position Overview

We are seeking a dedicated Physical Therapist to join our Home Health team serving New Braunfels, TX. This role is ideal for a clinician who values autonomy, one-on-one patient care, and the opportunity to help patients regain mobility and independence in their home environment.

Primary Responsibilities
Assess functional status and determine physical therapy needs; collaborate with the physician to develop the plan of care.
Provide treatment within Texas PT practice laws, following the plan of care and coordinating with the interdisciplinary team.
Observe and report patient responses and changes in condition to the supervising nurse and/or physician.
Deliver interventions to relieve pain, restore function, and maintain maximum mobility using accepted standards of practice.
Educate patients, families, and care team members on the plan of care, functional limitations, and progress.
Complete visits per the treatment plan, submit progress notes per policy, and maintain accurate, timely documentation.
Complete evaluations and develop PT plans of care within state guidelines.
Communicate evaluation results, goals, and projected frequency of care.
Provide care in diverse home environments regardless of patient background or circumstances.
Required Qualifications
Current Physical Therapy Licensure in Texas
Valid Driver's License, vehicle insurance, and reliable transportation
Must have Home Care Home Base experience
Must have home health experience
Must have experience treating post-operative orthopedic patients
