Overview
Select Health is a community health plan serving more than 1 million members. Their lines of business include Medicare, Medicaid, FEHB, Marketplace Qualified Health Plans, and fully funded and self‑funded Commercial plans.

Role Summary
The Behavioral Health Care Manager works with Select Health members, their families, and a multidisciplinary care team in a collaborative process of assessment, planning, facilitation, navigation, coordination, evaluation and advocacy to ensure members have access to benefits, resources, and services that meet comprehensive medical, behavioral, and social needs while promoting self‑management, quality, and cost‑effective outcomes.

Sign‑on Bonus
$2500 sign‑on bonus available for external applicants with at least 1 year of experience.

Work Schedule & Compensation
Schedule: Monday-Friday 8:30 AM-5:00 PM, 40 hours per week.

Pay Range: $39.16 - $60.42 per hour (exempt).

Key Responsibilities

Collaborate with physicians and other team members to improve health of patients with chronic or complex conditions.

Educate patients and caregivers to manage their health care needs.

Facilitate communication, coordinate services, address barriers, and promote optimal allocation of resources while balancing clinical quality and cost.

Assess complex mental health needs, develop, document and implement care plans that address social, physical, mental, emotional, spiritual and supportive needs.

Identify members who cannot adhere to a medical plan or who require complex assistance; enroll and follow them in longitudinal care management services.

Follow established care model or program policies to assess, establish and maintain individualized plans of care within the benefit plan.

Ensure plans are evidence‑based and consistent with referral, payer contract or established care processes.

Maintain records per regulatory requirements such as NCQA, CMS and State EQRO standards.

Coordinate and facilitate communication with behavioral health specialists, facilities, member/family/representative, health‑care team and other relevant parties through interdisciplinary team meetings or other coordinated communication.

Contact patient at intervals to determine if goals are being achieved and remain appropriate.

Determine variances and refer patients to a more comprehensive level of care when indicated.

Assess, consult, refer and/or deescalate mental‑health crisis remotely and in real time.

Minimum Qualifications

CSW or equivalent master’s degree in behavioral health or psychology field.

Valid State Licensure required in Utah with flexibility to obtain license in other states within the Select Health Region.

Demonstrated experience in care management/navigation or related field including utilization management, discharge planning, managed care, health promotion, health coaching, behavioral health, quality, or patient educator roles. Strong written and verbal communication skills.

Excellent written, verbal and conflict‑resolution communication skills.

Intermediate computer software and hardware skills, ability to customize settings, use multiple monitors and troubleshoot internet and applications independently.

CSW hired or promoted into this role must have or obtain an LCSW within one year of hire or promotion.

Preferred Qualifications

LCSW.

Three years of experience in clinical practice within an ambulatory care setting, community health or home care.

Experience working with third party payers.

Ability to work independently and flexibly in a rapidly changing environment.

Experience working successfully in a remote environment using advanced Microsoft Suite, including Teams (chat, whiteboard, task tracking) and Outlook.

Minimum 1‑year experience in care management/navigation or a related field, including utilization management, discharge planning, managed care, health promotion, health coaching, behavioral health, quality, or patient educator roles. Strong written and verbal communication skills.
