LPN/RN- NEEDED HOME HEALTH SHIFT CARE

Category: Skilled Nursing. Shift: Day. Department: Medical/Surgical Unit-3P1‑Trauma Care. Sign‑on bonus available. Responsibilities Guide practice by ANA Standard for Professional Nurse and Code of Ethics.

Maintain knowledge of professional nursing principles, practices, concepts, and procedures as applied to the patient population served.

Record patient medical history and symptoms, perform diagnostic tests, analyze results, operate medical equipment, and administer treatments and medications.

Establish and contribute to a care plan, including activities such as medication administration, IV line management, therapies, treatments, patient observation, and consulting with physicians and other clinicians.

Educate patients and families on illness or injury management, home care, diet, nutrition, exercise, and self‑administration of medications.

Provide direction to licensed practical nurses and nursing aides and delegate appropriate responsibilities.

precept new hires and students; Collaborate with multidisciplinary team to assess, plan, implement, and evaluate patient care delivery.

Engage in evidence‑based practice, research, and quality improvement initiatives.

Essential Functions Collect health data systematically, prioritize based on patient condition, and involve families.

Use assessment data to formulate diagnoses and develop, implement, evaluate, and modify individualized, family‑centered, culturally relevant care plans.

Initiate treatments, medications, emergency measures, and resuscitative procedures per policies and standing orders.

Provide holistic care addressing physical, emotional, spiritual, and social‑cultural needs.

Document observations, interventions, therapeutic measures, monitoring data, and other relevant information.

Continuously evaluate care quality and effectiveness to improve patient outcomes.

Collaborate with peers, mentors, and other health‑care providers to foster continuous improvement.

Identify and implement evidence‑based changes to nursing practice for better safety, effectiveness, and cost‑efficiency.

Education Associate’s or Diploma in Nursing.

All ADN or Diploma hires must complete a BSN within 60 months of hire or have 5+ years of RN experience at hire.

Bachelors in Nursing (BSN).

Experience Minimum 1 year RN experience for all positions, including float roles.

Licenses and Certifications Registered Nurse license in Arizona or applicable Compact state.

Unit‑specific certification as required.
