• Partnering with nephrologists, dialysis teams, and interdisciplinary care providers to deliver comprehensive dialysis care, address patient concerns, and ensure accurate, timely clinical documentation.

Managing chronic kidney disease patients across outpatient settings by conducting assessments, developing treatment plans, and coordinating ongoing care in collaboration with physicians.

Facilitating seamless transitions of care through annual wellness visits, post-hospital follow-up, medication reconciliation, and patient education to improve outcomes and reduce readmissions.

Providing advanced clinical care across multiple settingsincluding physician offices, skilled nursing facilities, rehabilitation centers, dialysis units, and on-call environmentsthrough diagnostic evaluations, treatment planning, and evidence-based interventions.

Prescribing medications and therapeutic treatments within scope of practice while delivering compassionate, patient-centered care and maintaining the highest standards of professionalism and clinical excellence.

Master's degree in nursing or related field, or equivalent combination of education and experience (4-10 years)

Current Arizona RN license with Nurse Practitioner (NP) or Physician Assistant license

Proficiency in Microsoft Office and experience with ICD-9 and CPT coding

Strong organizational skills, flexible schedule availability and reliable transportation for travel between sites
