Full & Part Time Direct Support Professional - Overnights

Direct Support Professional - PRN at Horizon Health and Wellness . This role involves flexible, as‑needed shifts during the holiday season, with additional opportunities thereafter.

Provide essential care and support services to individuals with intellectual and developmental disabilities.

Assist with community integration and inclusion, offering guidance to individuals and their support networks.

Create meaningful opportunities for participation in community activities.

Ensure the health, safety, and overall well‑being of residents through proper supervision.

Pick up shifts as needed during the holiday season and beyond.

High School diploma or GED.

Ability to prepare required paperwork and document activities within contractually required timelines.

Typing proficiency and working knowledge of computers and electronic clinical record keeping.

Ability to successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, Prevention/Support, and Direct Care Worker (training provided if not possessed).

Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance.

Willingness to work nights, weekends, holidays, and a varied weekly schedule.

Bilingual English/Spanish or English/Sign Language.

Two years of experience in a person‑centered community integration program assisting persons with developmental and/or psychiatric disabilities.
