OhioHealth is seeking an LPN to join the Patient Advice and Care Team (PACT) at our primary care practices. The position supports a centralized medication refill call center, delivering nursing care, health assessments, and coordination across care teams in a fast-paced environment.
Responsibilities include documenting patient data, initiating care plans, and contributing to process improvements while acting as a patient advocate. Monday-Friday, 9:00 a.m. to 5:30 p.m., no weekends or holidays.
