Caseworker - Social Work

As a Social Worker, you will play a critical role in ensuring the well-being of our residents by providing emotional support, counseling, and advocacy.

In keeping with our organization's goal of improving the lives of our residents, the Social Worker will manage the day-to-day operations of the Social Services Department and be responsible for the overall administration, coordination, and evaluation of the social services function. The ideal candidate will have a strong background in social work, excellent communication skills, and a passion for working with seniors.

Keep up-to-date evaluation documentation on each resident's activities at the facility, complying with Federal, State, and Local regulations.

Attend scheduled care conferences and identify and interpret psychosocial needs of residents for inclusion in the care plan.

Assist with the coordination, intake, admission, and resident transfer within the facility.

Coordinate the inspection of residents' belongings to ensure they are properly labeled and inventoried.

Attend Interdisciplinary Team and Unit Care conferences, assess residents for psychosocial groups, and supervise implementation and documentation.

Mediate issues that arise among residents, families, and staff.

Provide linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs.

Develop a comprehensive discharge plan in collaboration with residents, residents' families, multidisciplinary staff, and referral agencies.

Provide informal counseling when needed to uncover any problems that might interfere with residents' socialization and participation in activities.

Establish, develop, maintain, and update filing systems for the Social Services Department.

Operate the Social Services Department within budgetary guidelines and limitations.

Complete annual performance reviews of all subordinate staff, provide guidance and education, and counsel and discipline as necessary.

Report all hazardous conditions, damaged equipment, and supply issues to appropriate persons.

Bachelor's degree in social work or similar field

Two years' experience in a social worker role, preferably in a health care setting
