Job Description Pharmacist – Home Infusion
Schedule: Monday–Friday, 9:00 AM–4:00 PM
Requirements
Bachelor of Science in Pharmacy required; PharmD preferred
Active Registered Pharmacist license in state(s) of practice
Home care pharmacy experience preferred
IV/infusion compounding experience preferred
Key Responsibilities
Review, prepare, and dispense prescription medications for home care patients
Evaluate and monitor drug therapies and patient medication compliance
Provide medication counseling regarding dosage, administration, side effects, storage, and interactions
Compound medications and prepare sterile/infused solutions following proper procedures
Maintain accurate, confidential patient and pharmacy records
Identify drug-related issues and communicate with physicians and healthcare providers
Collaborate with nurses, pharmacists, pharmacy interns, and other healthcare professionals
Maintain appropriate inventory of pharmaceutical and medical supplies
Supervise pharmacy technicians and clerical staff as needed
Educate patients on proper use of medications and medical devices
