Job Overview The Wellness Nurse is responsible for monitoring the health and well‑being of our residents and providing leadership support and assistance with resident care and services. Responsibilities include overall well‑being of residents, assisting in training and monitoring of medication care administration, communication with resident support services and families, and maintaining clinical quality assurance while demonstrating Sunrise Senior Living’s mission.
Responsibilities Coordination of Health Needs Identify ongoing needs and services of residents to promote the highest quality services.
Ensure proper follow‑through and participate in evaluation and documentation for residents with a change in health care services and status.
Assist the Resident Care Director in conducting resident evaluations and data gathering at move‑in, 30 days post‑move‑in, semi‑annually, and at any change in status.
Conduct monthly wellness visits for all residents.
Monitor and follow up with residents who have experienced a fall, accident, or incident; document and report as required.
Document all pertinent information in the resident wellness file.
Maintain communication with residents’ families or responsible parties to ensure medical needs are met.
Contact the resident’s attending physician when necessary and/or upon family request.
Ensure weights and vital signs are obtained monthly for each resident.
Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.
Maintain medical supplies and emergency kits for the community.
Provide clinical support and assistance to community team members as needed.
Understand and follow infection control practices.
Partner with the Resident Care Director and community team to promote infection control standards, including testing, training, and skills checks.
Practice proper methods of attending to and disposing of blood‑borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
Medication Programs Monitor each resident’s medication and treatment profile regularly to ensure accurate administration and documentation.
Transcribe physician orders into the Medication Administration Record / Treatment Administration Record (MAR/TAR), including initials, date, time, and date faxed to pharmacy.
Act as liaison for pharmacy services to ensure efficient service for residents.
Complete Medication Observations and skills checks for medication care managers at the direction of the Resident Care Director.
Quality Assurance and Regulatory Compliance Demonstrate knowledge in key quality improvement areas: Resident‑Centered Care Model, APIE, Quality Care Indicators and Outcomes.
Actively participate in Sunrise Quality Assurance Meetings and Clinical Meetings.
Show knowledge of federal, state/provincial, and local regulations: Resident Rights, Assessment/Evaluation processes.
Review Physician Reports of new residents as directed.
Order medications and equipment as directed by the Resident Care Director.
Know Sunrise practices and guidelines in resident care programs, including skin, nutrition, behavior, falls, and bowel/bladder.
Assist in implementation, training, and monitoring of documentation related to resident’s clinical health and wellness.
Report changes, concerns, and solutions to the Resident Care Director appropriately.
Contribute to engagement programs and processes: team member engagement survey and planning workshops.
Maintain compliance with required training and state/provincial or other regulatory authorities.
Experience and Qualifications Graduate of an approved college/school of nursing.
Maintains a current state/provincial license as a Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN).
Minimum of one (1) year experience in home health, assisted living, hospital, or long‑term care environment.
Knowledge of nursing practices, techniques, and methods applied to resident care and regulations.
Proficiency in computer skills (Microsoft Office, Sunrise applications) and ability to learn new applications.
Availability to work weekends, evenings, and flexible hours to meet peak service delivery.
Pre‑Employment Requirements Completion of a drug test (excluding marijuana) and required testing (e.g., COVID‑19 and influenza vaccination as mandated by applicable laws and authorities.
31.35 – $41.90 per hour.
No bonus or commissions.
