Outpatient Counselor - Health Clinic

Helping People. Changing LivesWe are seeking a dedicated and knowledgeable Behavioral Health Counselor to join our SLO-Hub program. Under the direction of the SLO-Hub Program Supervisor and in collaboration with the contract clinical supervisor, you will provide assessment and therapeutic intervention for clients. A two-year Master's degree from an accredited institution in a behavioral health related field. Registered Intern preferred.Two years of professional behavioral health therapy experience in working with homeless individuals and/or individuals struggling with substance abuse and/or behavioral health issues preferred.Preference to individuals who have already achieved LMFT, LCSW, or other behavioral health licensure.Fluent in two or more languages, including one that supports the needs of the program and community preferred.As the Behavioral Health Counselor you will:Conduct preliminary behavioral health evaluations and complete required GPRA assessmentsDevelop and implement effective, individualized treatment plans using appropriate therapeutic modalitiesProvide counseling and therapy services in clinic, recovery housing, community settings, and non-traditional environments; may include evenings or weekendsCollaborate closely with internal team members (Case Managers, Peer Support Specialists) and external community partnersMaintain accurate, timely case documentation, client records, and required statistics while ensuring confidentialityCollect and submit grant-required data and reports within established timelinesMonitor client progress through ongoing assessments and follow-up servicesStay current on best practices for working with dual-diagnosis clients through training and professional developmentMaintain knowledge of community resources to support client treatment and recoveryParticipate in program evaluation activities, community meetings, and required reportingAdhere to organizational policies, safety practices, and CAPSLO's mission and valuesPlease note this job advertisement is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
