Our mission is to empower patients and be leaders in Obesity Medicine driving impact at a national scale. We are seeking a dedicated and collaborative Physician specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. This new role is designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in supporting endocrine Advanced Practice Providers (APPs) in providing high-quality patient care. The physician will provide direct clinical services and structured supervision for up to three Endocrine-Specialized APPs, ensuring safe and effective management of patients with complex co‑morbidities, including Chronic Kidney Disease, End‑Stage Renal Disease, and cardiovascular conditions. The physician will also help integrate the use of Continuous Glucose Monitors and insulin pump therapy.

This role includes participation in a rotating after‑hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule during on‑call coverage. The position is full‑time, based in a remote telemedicine setting, and provides protected time for supervision, teaching, and administrative duties.

Provide direct patient care in a telemedicine clinical setting, focusing on evidence‑based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients using insulin pumps and/or continuous glucose monitors.

Collaborate with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes, to co‑manage a shared patient panel.

Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.

Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.

Prescribe non‑controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs.

Conduct weekly 30‑minute collaborative meetings and 30‑minute asynchronous communication time with each assigned Endocrine‑Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship.

Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine.

Participate in team‑based care initiatives and contribute to a culture of collaboration and high‑quality care.

Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.

Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)

Required: Active and unrestricted medical license in at least one of the following states: Required: Current or prior credentialing with insurance payers

Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy.

Minimum of 5 years as an attending physician in endocrinology.

Demonstrated competency and comfort in managing patients with complex co‑morbidities, including CKD, ESRD, and CAD/CV conditions.

Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens.

Strong interest in mentoring, teaching, and collaborating with Endocrine‑Specialized APPs and Registered Dietitians specializing in diabetes.

Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.

Adherence to evidence‑based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity.
