CNA - Hospice at Home Care - Part-Time

Job Description This position is a part-time, hourly, non-exempt position
Work Schedule: Monday-Friday, 8am-5pm (40 hours/week)
Location: travels to homes and facilities within a 30-mile radius from our Asheboro office
This position provides personal care and home management tasks at patient’s residence, under the supervision of a RN Case Manager.

Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
Reports changes in patient’s condition, concerns about patient and/or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary.
Works collaboratively with the IDG and other departments in achieving patent care goals.
Documents patient care according to agency policy and procedures; Responsible for professionally managing time, people, medical supplies, and technology.
Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members.
Teaches and/or reinforces home safety, energy conservation and sanitation.
Provides light housekeeping tasks.
Participates in staff training and education programs.
We are allowed to do more one-on-one care with patient and family than at hospitals or nursing homes. We have time to sit and talk to the patient and families, help them go through whatever they are going through, and give them they respect and care they deserve.
We have the ability to be with people in their last hours/days, be a constant for them, and show them that we care about them - some patients may not have family.

In hospice care, you are dealing with grief. Many of our employees find that what helps them on those hard days is to find joy in the fact that they were able to be there to comfort that patient, help them feel cared for, and make a difference in the time they had with them.
