Hospice RN Case Managers

As a RN Case Manager, you will:

Visit with patients in their homes or nursing facility setting, providing assessment, teaching and skilled care

Work with and receive support of other professionals to problem solve and update the patient's plan of care

Requirements:

Valid Registered Nurse license in the state of Missouri

At least two years of hospice/palliative care experience preferred

Excellent computer skills

Excellent organizational and communication skills a must

Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance
