About the Role
Conduct thorough patient evaluations in both inpatient and outpatient environments, crafting personalized treatment strategies
Carry out targeted physical therapy interventions that prioritize mobility, strength, balance, and functional independence
Work alongside a multidisciplinary team including nurses, physicians, case managers, and rehabilitation professionals to ensure optimal patient outcomes
Inform patients and families about therapy goals, discharge planning, and at-home continued care
Maintain accurate documentation of assessments, interventions, and progress per facility protocols and standards
Engage in care team discussions and collaborative rounds to promote synchronized patient care
Education
~ Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited program

Certifications
Must possess valid state licensure as a Physical Therapist
CPR certification is preferred
Specializations (neuro, geriatrics, etc.) are advantageous but not mandatory
Skills
Previous experience in a clinical setting is highly favored
