License Practical Nurse, Second Shift - Home Healthcare

Job Summary: The Registered Nurse (RN) or Licensed Practical Nurse (LPN) in this Long Term Care Center role is responsible for assessing, planning, and implementing resident care under the direction of the Unit Manager. The nurse supports the mission and nursing philosophy of the facility, providing skilled nursing care tailored to the needs of long-term care residents. The position requires a current nursing license in Montana and the ability to effectively communicate and collaborate within the care team.

Responsibilities:

Participate in resident care assessment, planning, and implementation.

Provide skilled nursing care consistent with long-term care standards.

Support the mission and nursing philosophy of the care center.

Collaborate with interdisciplinary team members to ensure quality care.

Maintain accurate and timely documentation of resident care and condition.

Communicate effectively with residents, families, and healthcare providers.

Required Skills & Certifications:

Graduation from an accredited Registered Nurse or Licensed Practical Nurse program.

Current and active RN or LPN license to practice in the State of Montana.

Ability to learn and retain pertinent clinical and procedural information.

Effective communication and interpersonal skills.

Access to a current telephone number for communication purposes.

Preferred Skills & Certifications:

Experience in long-term care or similar healthcare settings.

Strong teamwork and collaboration skills within interdisciplinary teams.

Special Considerations:

Applicants must reside beyond a 250-mile radius to be considered for traveler positions.

Scheduling:

Shift schedules and assignments will be determined by the facility and Unit Manager.
