This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
• n Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you \n
• n Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status. \n Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines \n
• n Work independently, but never alone \n

• n Active Licensed Practical Nurse (LPN) license in the state you work \n
• n Two years of LPN experience and the clinical expertise to deliver quality patient care \n
