We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. EPIC experience

Experience with medication reconciliation for discharge medication lists

Experience gathering prior-to-admission home medication lists from patients, families, or pharmacies

Experience with pharmacokinetic dosing management and monitoring

Experience with anticoagulation dosing management and monitoring
