Hospice Triage Nurse - Overnight

Answer hospice calls during nights, weekends, holidays, and emergencies (e.g., Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.

Talk with patients and caregivers to assess needs and provide support over the phone.

Communicate with doctors about patient updates and obtain new care orders if needed.

Provide instructions and guidance based on each patient’s care plan.

Coordinate after-hours needs such as equipment, supplies, and medications.

Work with facility staff to support their patients during off-hours.

Document all calls, care updates, and decisions clearly and accurately.

Ensure all care and procedures follow state and federal rules.

Graduate of an accredited nursing school with current licensure to practice in the state of operation.

1 year hospice nursing experience required; Experience in hospice or similar setting preferred.

Nursing Compact RN license required.
