Title: Pharmacist
Job Type: PRN availability Monday- Friday 8 am- 6 pm, averaging 5-6 hours per day

Temple Rehabilitation Hospital is a 36-bed inpatient acute rehabilitation facility located at 23621 SE H.It's designed to help individuals recover from serious injuries or illnesses that have significantly impacted their ability to function independently.

Intensive Therapy Model : Patients receive at least three hours of physical, occupational, and/or speech therapy per day, five days a week, under the supervision of a rehabilitation physician.
State-of-the-Art Facility : Offers modern therapy equipment and private rooms to support a comfortable and focused recovery environment.

Provides safe, timely, and effective pharmaceutical care. Verifies and dispenses medications, oversees medication-use processes, reviews drug therapy for appropriateness and safety (including anticoagulant monitoring), supervises pharmacy technicians, and collaborates with the care team on medication-related quality and performance improvement.
selection, storage, ordering/transcribing, preparation/dispensing, administration support, and monitoring); identify risk points and drive continuous quality and performance improvement with clinicians.
Verify and approve work performed by Pharmacy Technician(s); provide coaching and feedback.
promote rational, evidence-based therapy and document interventions.
Perform anticoagulant monitoring and communicate dose/therapy recommendations per protocol or provider order.
Ensures compliance with hospital policies, state/federal regulations, and accreditation standards. inventory, controlled substances, medication safety reports). Utilizes pharmacy information systems and automated dispensing technologies as required.
Graduate of an accredited school of pharmacy; Current state license to practice pharmacy.
Experience: Two (2) years of hospital pharmacy experience preferred.
proficiency with pharmacy/EMR systems and inventory control.
