We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Lake Forest area. This position is eligible for a $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
Work independently, but never alone
Travel to patient homes within assigned territory

PT Qualifications
Active Physical Therapist, PT license in the state you work
One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
