Direct Support Professional DSP Caregiver Companion

Job Description Job Overview: The Home and Community Based Services Specialist plays a vital role in assisting individuals to maintain their independence and enhance their quality of life by providing a variety of supportive services in their homes and communities. The specialist collaborates with clients, families, and other service providers to develop and implement personalized care plans.
Client Support:
Provide direct support to clients, assisting with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as needed.
Offer companionship and engage clients in social and recreational activities to promote overall well-being.
Documentation:
Maintain accurate and up-to-date records of client assessments, care plans, and service delivery.
Document progress, outcomes, and any changes in clients' conditions or needs.
Assessment and Planning:
Conduct comprehensive assessments of clients' needs, preferences, and goals for living independently.
Collaborate with clients and their families to develop individualized care plans that outline the necessary services and support.
