Are you a Risk Adjustment and HCC expert? y ou will train, support, and perform annual wellness visits in assigned IPA outpatient clinics. The visits will focus on Medicare documentation guidelines and HEDIS/Quality measures. You will also focus on clinic workflows that align with Value-based Care delivery models. Improve Quality and Care management.

Collaborate with population health and the quality department to improve workflows

Close quality gaps, improve identification and capture of HCC codes, and improve medication adherence.

Providing elbow support and training to IPA Physicians and Advanced Practice Providers on Medicare documentation guidelines, HEDIS/Quality measures, and clinic workflows that align with Value Based Care delivery models.

Successful completion of a Nurse Practitioner/Physician Assistant program with Board Certification

Knowledge of HEDIS quality indicators
