SLP - Speech and Language Therapist
TactStaff
Seeking a Speech Language Pathologist to provide expert speech therapy services within a home health setting. Assess patients' speech, language, and swallowing capabilities to develop personalized treatment plans.
Deliver direct therapy interventions to patients in their homes, focusing on improving communication skills.
Document patient progress, treatment sessions, and clinical notes accurately in EMR systems.
Collaborate with caregivers and healthcare teams to coordinate care and optimize patient outcomes.
Manage a caseload of 5-6 patients daily, ensuring timely and effective therapy sessions.
Maintain compliance with travel, documentation, and licensing requirements.
Hold a valid state licensure as a Speech Language Pathologist.
Possess at least 1 year of recent adult home health or hospice experience.
Be willing to travel up to 75 miles from the assigned care center.
