Mileage reimbursement at .Job Summary As a Physical Therapist (PT), you are key to helping us provide superior patient care. You will work to provide skilled Physical Therapy services in accordance with the client’s plan of care to include helping clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.

We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.

Under the direction of a Registered Physical Therapist (PT) provide client services which have been delegated by the PT- which includes: assist in evaluation and development of a rehabilitative plan of care and in periodic re‑evaluation as required

Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted within 72 hours, also prepares periodic summary reports

As directed, perform routine strengthening and therapeutic exercises and assists the client with the use of self‑help devices

Together with the PT, instruct the client and family and assists in teaching exercise programs to the client and family

Consult with PT regarding the outcome of home visits and reports physical findings in written form

Participate in case conferences, therapy staff meetings, agency activities and in‑service programs as necessary

Qualifications Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants

Must have at least one year of institutional/home health experience

Reliable transportation, proof of valid car insurance, and a valid driver’s license
