Schedule
AM 5 × 8 hr shifts, Monday-Friday, 40 hours per week.
Job Description
We are seeking a skilled Home Health Physical Therapist to provide quality care in a home health setting. The ideal candidate will be responsible for delivering physical therapy services to patients in their homes, conducting evaluations, and developing treatment plans tailored to patient needs.
Responsibilities
Conduct initial evaluations and assessments of patients.
Develop and implement individualized treatment plans.
Provide ongoing physical therapy interventions and documentation.
Collaborate with healthcare team members to ensure comprehensive care.
Maintain accurate and timely documentation in HCHB.
Required Experience / Certifications / Licensure
Active Physical Therapist (PT) license.
Home Health and OASIS experience required.
Experience with Homecare Homebase documentation preferred.
Ability to travel within a 60-mile radius from the facility address.
