Minimum Requirements

Associate, Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.

Active Florida Physical Therapy license required prior to the start of the assignment.

Home health care experience is required, including documentation of services provided in patients' homes.

Familiarity with home health travel routines and traveling between patient locations within a shift.

BLS certification from AHA or ARC may be required.

Preferred: HCHB (Homecare Homebase) EMR experience.

Assignment Details

Facility type: Home Health. Shift: Days. Shift hours: 8:00 AM to 5:00 PM. Work days: Monday through Friday. Weekly guaranteed hours: 32 to 40. Productivity expectation: 30 points. Coverage area: Indian River County, St. Lucie County, and Martin County. Travel between patient homes is required. Mileage between visits will be reimbursed. On-call/weekend requirement: None. Required interview format: Teams. Dress code: Scrubs or slacks; any scrub color acceptable.
