Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Ability to collect data from initial assessment and other clinical tools to adequately facilitate and provide treatment planning; provide individual, family and group therapy services with an emphasis on the program specific population.

SHIFTS AVAILABLE:

Bachelor’s Degree in counseling or the social sciences required.
Must comply with agency policies and procedures.

Must be able to gain access to all types of buildings and residences regardless of the structures’ handicapped accessibility or lack thereof.
Must maintain current phone number and mailing address on file with the human resources office.

Provides treatment planning for assigned clients.
Reviews treatment plan goals and short-term objectives with the client at each individual treatment session, tracks progress towards meeting those goals, and records client responses in client’s own words in the clinical record.
Works with client to develop an interim plan for working towards achievement of goals both outside of the session and at the next schedules session and records them in the progress notes.

Provides individual, family, group therapy, and case management as appropriate under the supervision of a Mental Health Professional.
Coordinates treatment services with primary care providers and other relevant agencies; Outlines significance of working with primary care, other relevant agencies, and significant others at the time of development of the treatment plan evidenced by treatment plans and progress notes and release of information.
Meets direct service productivity requirements as assigned by the Clinical Manager of Adult Services.

Maintains records and reports in accordance with agency policies and procedures.
Completes progress notes and data verification to be available to MIS according to agency policy and procedure.
Completes treatment plans, crisis plans (as applicable), reauthorization of services, progress notes according to agency policy and procedure.
Completes other forms as required by agency policy and procedure.

Participates in supervision with a Mental Health Professional, consultation, and training at the direction of the Clinical Manager of Outpatient Services.
Meet weekly with the Clinical Manager of Outpatient Services for supervision and to discuss support staff needs.
Completes all trainings as outlined on Annual Training Plan within the timeframe set forth by the agency.
Participates in quality improvement activities outlined by the agency.

Coordinate client data processing procedures.
Arrange schedule to allow time to for coordination of care and return phone calls within one business day (i.e., answer questions for staff related to client data, communication to support integrated care, follow up with client/family, etc.)

Cascade is an eligible employer for the Public Service Loan Forgiveness (PSLF) Program. Cascade is also an approved site for Washington Health Corps – State Health Program, Behavioral Health Program, and the Federal Health Program
Self-Care, time away from work. Start off with 13 hours of paid time off (PTO) earned monthly, and 11 paid holidays plus 16 hours of personal holiday. PTO and Personal Holiday are available after 90-days of employment.
