Nurse Practitioner – Primary CarePhysician Care Centers (PCC) is a growing healthcare MSO with 70+ value-based primary care clinics serving patients across Florida, Texas, Georgia, South Carolina, and Nevada. With a team of 500+ professionals, we're committed to delivering exceptional care and improving patient outcomes.We're seeking a Nurse Practitioner – Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, you'll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention. Your clinical expertise and patient-focused approach will support our mission of delivering high-quality primary care in a compassionate and efficient environment.What You'll Do:Perform comprehensive physical examinations and health assessments.Prescribe medications and therapies in accordance with state regulations.Provide preventive care, including immunizations and routine screenings.Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease.Educate patients and families on disease management, lifestyle modifications, and preventive care.Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies.Coordinate care with specialists and other healthcare professionals.Document patient encounters accurately and timely in the electronic health record (EHR).Participate in quality improvement initiatives and population health management.What We're Looking For:Master's or Doctorate degree in Nursing (MSN or DNP).Graduate of an accredited Nurse Practitioner program.Current Registered Nurse (RN) license.Current Nurse Practitioner license in the practicing state.National board certification (AANP or ANCC) – Family NP or Adult-Gerontology NP.DEA registration (if prescribing controlled substances).Proficiency with electronic medical records (EHR/EMR).Cultural competency and a patient-centered care focus.Outpatient primary care clinic.May include community health centers, private practices, or hospital-affiliated clinics.Monday–Friday, 8am–5pm.Apply today and help us build the future of patient care!
