Care Management RN - Full-time
Lifespan
Brown University Health seeks a Case Manager to coordinate care and expedite patient services. Reports to Manager or Director of Case Management and works with the clinical team across the care continuum, including preadmission and discharge planning.
Full time, part time per diem and job share schedules available. Responsibilities include collaborating with medical staff, identifying high‑risk patients, developing discharge plans, and ensuring smooth handovers.
