Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Home-Based Primary Care Program in Baltimore, Howard County, and Anne Arundel County in Maryland. This growing program focuses on high needs geriatric home-limited patients and provides high-touch, comprehensive primary care. Collaborating with the Medical Director and other team members, the Nurse Practitioner (NP) will provide medical care for a small panel of home bound and medically complex older adults. The NP will be responsible for evaluating and treating patients receiving primary care at home, coordinating their care with other team members and specialists, and participating in weekly inter-disciplinary team meetings. General medical experience with geriatric, medically complex and high acuity patients desired
- Part time, 20 hours clincial and 4 hours admin
- Must have a working vehicle to travel to patient's homes.
- 3 years’ experience preferred in primary care or acute/urgent care.
- Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Remington practice for the remaining .Johns Hopkins Community Physicians serves Maryland, Virginia and Washington DC with over 40 locations.
Part -Time- 20 hour clinical and 4 hours admin
Admin time may be remote
Night call once per month
Productivity model includes bonus eligibility
