Licensed Practical Nurse Weekends - Hospice

RN Weekend Baylor (36 hrs, Fri, Sat, Sun)Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last three years, our community-based, mission-driven hospice and palliative care organization seeks a Weekend Nurse to join its expanding On-Call team. Travel to provide care in patients' private homes and residential facilities throughout Greater Boston and parts of MetroWest. This position is eligible for a $5000 sign-on Bonus.The Weekend Nurse is an integral part of an interdisciplinary hospice team responsible for caring for patients at the end of life. Makes home and hospital visits, provides information sessions, evaluates new patients, facilitates admissions to hospice services, and coordinates care planning. Communicates with an interdisciplinary hospice team about changes in patients' needs and outcomes of interventions.Schedule and Weekly Hours:Fridays, Saturdays, and Sundays, from 9 am to 9 pm (36 hours/week).Education, Skills and Experience:Licensed Registered Nurse (RN) in Massachusetts or Licensed Practical Nurse (LPN).
