Licensed Practical Nurse ( LPN ) - Home Care Night Shift - URGENT NEED

Location & Shift Seabrook Village by Erickson Senior Living. Currently hiring full‑time night shift (11p-7:30a).

Position Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN).

Responsibilities Developing relationships with residents and families using a consistent caregiver approach.

Supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.

Assessing, observing, and evaluating residents’ clinical condition, documenting findings in the resident electronic medical record and notifying the provider and/or RN when necessary.

Updating or modifying care plans as needed to reflect changes in a resident’s care and service needs.

Supervising and leading care associates.

Administering medications timely, accurately, and according to orders.

Participating in meetings related to resident outcomes and care.

Responding to residents, families, or responsible parties in a timely and professional manner.

Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.

Other duties as assigned by RN clinical leader.

Qualifications Minimum of 1 year experience in assisted living, home health, or long‑term care (preferred).

Licensed by State as a Licensed Practical Nurse.

CPR certification required.

Passion to care for seniors and be a strong resident advocate.

Experience with electronic medical records preferred.

Specific state regulations and requirements may apply; precedence over listed requirements.

Compensation Base pay starting $36+ and $2 per hour shift differential for night hours.
