This role also plays an important part in obtaining resident weights, assisting with exercise programs, and helping residents achieve their highest practicable level of functioning.Key ResponsibilitiesAssist residents with restorative exercises and strengthening programs as directed by nursing and rehabilitation staffSupport therapy and nursing departments with weight management programs and routine resident weightsAccurately obtain, record, and report resident weights according to facility protocolEncourage participation in exercise, mobility, and out-of-bed activitiesAssist with transfers, ambulation, range-of-motion exercises, splinting, and restorative programsAssist residents with dining, bathing, dressing, toileting, and other activities of daily living as neededReposition residents regularly to promote comfort and skin integritySupport residents in maintaining personal hygiene and cleanlinessAttend restorative nursing meetings and collaborate with therapy and nursing teamsComplete accurate documentation and charting in PointClickCare (PCC)Maintain a positive, respectful, and therapeutic environmentPromptly report changes in resident condition or concerns to nursing and rehabilitation leadershipPrepare residents for appointments, meals, activities, and treatmentsAssist with skin protection measures and pressure-relieving devices as directedFollow all facility policies, infection control procedures, and safety standardsQualificationsEducation & ExperienceHigh school diploma or equivalent requiredOne year of long-term care experience preferredExperience with PointClickCare (PCC) preferredStrong communication, teamwork, and organizational skillsLicenses & CertificationsCurrent South Carolina Certified Nursing Assistant (CNA) certification in good standingRestorative Nursing Assistant (RNA) training or certification requiredMust maintain all required continuing education requirementsCPR certification if required by licensure or facility policyMust remain in good standing with all applicable licensing and certification boardsWhat Makes You a Great FitPassion for helping residents maintain strength and independenceStrong interest in rehabilitation and restorative nursingCompassionate, patient, and encouraging personalityDependable with excellent attention to detailTeam-oriented and eager to collaborate with therapy and nursing staffApply TodayJoin Chandler Creek Post Acute and become part of a dedicated rehabilitation team focused on helping residents achieve their highest level of independence.
