Nursing - Registered Nurse, Hospice RN, Home Health RN - Travel

Registered Nurse – Home Health (Hospice) travel assignment in Rutland, Vermont. Weekly pay: $2,488–$2,655 per week. Shift: 5x8 Days. Setting: Home healthcare – Hospice. This is a travel temporary assignment.

• Contract duration: Shift: 5x8 days
• Home healthcare – Hospice

Deliver professional nursing care to home-based patients using the nursing process; assess patient and caregiver needs and develop an evidence-based plan of care.

• Serve as Case Manager for assigned patients, actively managing caseloads and visit times in line with agency standards.
• Complete and maintain accurate OASIS documentation in accordance with agency policies and regulatory requirements.
• Implement the medical plan of care and nursing interventions; maintain infection control standards and timely documentation.
• Collaborate with LPNs and other disciplines to ensure the plan of care is followed and patient needs are met; Current, active RN license in Vermont (Vermont State Board of Nursing) or equivalent with eligibility to practice in Vermont.
• Prior home health and hospice experience preferred; strong assessment, care planning, and documentation skills.
• Proficient in high-tech nursing skills (IV use and maintenance, wound care including wound VAC, complex wound care) and infection control practices.
• Some on-call and weekend coverage required.
