How You'll Make An Impact In This Role
Strategic Partnership: Serve as the primary liaison between administration, physicians, and managed care partners.
Care Coordination: Direct treatment efforts while championing quality improvement and staff education.
Data Synthesis: Compile metrics and track outcomes to drive high-level process improvements.
Patient Advocacy: Manage specialized referrals and follow-up care for uninsured and under-insured populations.
Community Engagement: Foster vital relationships with community organizations to extend the continuum of care.
What Minimum Requirements You'll Need
Licensure / Certification / Registration:
Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
~ Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What Additional Preferences We're Seeking
~ Patient Navigator and Case Management experience preferred
