Job Description
The Pharmacist, compounds and dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration to other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use oversees the work of the support staff and documents any relevant intervention. The position is expected to meet state certifications, requires familiarity with standard concepts, practices, and procedures, and relies on experience and judgment to plan and accomplish goals.

Proficiency is developed in pharmacy information systems including the ability to troubleshoot and/or clarify orders as evidenced by the resolution of problems immediately and lack of complaints.

Clinical pharmacists are notified when an order is placed in the clinical queue.

Initiative is displayed to prevent, identify, and resolve pharmacy related patient care problems immediately.

Appropriate adult/geriatric/pediatric/neonatal patient care is provided after assessment and interpretation of patient data and needs.

Concise, applicable, comprehensive and timely responses to requests for drug information are provided to patients and health care providers as evidenced by lack of complaints.

Medications are correctly filled during the filling process. Supervision is provided at all times in accordance to established guidelines.

Medications are accurately weighed, measured and compounded utilizing appropriate tools and equipment as evidenced by lack or errors. Supervision is provided at all times in accordance to established guidelines.

Scheduled medications are correctly dispensed/returned to the Narcotic Vault 100% of the time as evidenced by reports.
