Provide One-on-One Care. Are you a Licensed Practical Nurse (LPN) looking for a rewarding role in home health or in-home care? At Americare Home Health, you’ll deliver personalized, one-on-one care that helps patients stay safe, independent, and supported in the place they call home.

We are seeking a Licensed Practical Nurse (LPN) to provide high-quality care in a home health setting, supporting patients in Americare communities and in-home environments. This role focuses on patient-centered care, coordination with care teams, and maintaining a safe, comfortable, and home-like environment that supports each patient’s physical, emotional, and social well-being.

Provide direct patient care in home health and community-based settings

Serve as a liaison between Americare Home Health and community teams

Assess patient needs and support individualized care plans

Implement nursing standards, policies, and care protocols

Monitor patient condition and report changes to the care team

Collaborate with physicians, nurses, therapists, and leadership

Provide guidance and support to caregivers and nursing staff

Basic knowledge of home health or senior living care

Graduate of an approved Practical Nursing (LPN) program

Current, active LPN license in the applicable state

Previous experience in home health, long-term care, or senior living preferred
