Pharmacist - Clinic Pharmacy (Full-Time)

Ambulatory Pharmacist
We're different than most primary care providers. The Ambulatory Pharmacist is a patient-centered clinical professional dedicated to optimizing medication therapy within primary care clinics. Operating under a collaborative practice agreement, this role focuses on reducing polypharmacy, enhancing medication adherence, and managing chronic diseases to improve patient outcomes and elevate quality performance metrics such as CMS Star Ratings and HEDIS scores. The ambulatory pharmacist partners closely with patients, physicians, advanced practice providers, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness.
Collaborates with healthcare providers to adjust medication therapies, interpret lab results, and support clinical decision-making within a protocol-driven framework.
Chronic Disease Management: Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications.
Lab Testing & Monitoring: Orders, interprets, and utilizes laboratory results to monitor medication, safety and effectiveness.
Administrative & Support Activities: Assists with physician consultations, provides accurate drug information, and supports the department's quality improvement initiatives.
Familiarity with appropriate local, state, and federal pharmacy-related regulations
Willingness to travel locally up to 50% of the time; flexible to work evening, weekends and/or holidays as needed
Fluency in spoken and written English
Doctorate of Pharmacy (PharmD) required
Valid pharmacy license in good standing Registered Pharmacist (RPh) required, Doctor of Pharmacy (PharmD) preferred
Pharmacy residency or ambulatory care certification (e.g. Certification in immunization and medication therapy management is a plus
Residency training or ambulatory care certification is highly preferred
The posted pay range represents the base hourly rate or base annual full-time salary for this position. This position may also be eligible for a bonuses or commissions.
