DAY Shift Registered Nurse - Hospice Care

The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness within a home health setting. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. The RN is expected to manage a caseload of patients, ensuring quality care and adherence to established protocols.

Provide skilled nursing care to patients in their homes.

• Perform patient assessments and develop individualized care plans.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Document patient care accurately and timely using electronic charting systems.
• Educate patients and families on disease management and self-care techniques.
• Active Registered Nurse license.
• Minimum of 1 year of skilled home health experience.
• Driver's license, car insurance, and reliable transportation.
• COVID vaccination and booster.

The facility can call off up to one shift per pay period.

• Local candidates within a 50-75 mile radius are accepted.
• Occasional travel radius of up to 60 miles per day may be required.

8-hour day shifts.

• The facility cannot accommodate 4x10 or 3x12 schedules.
