Licensed Practical Nurse

The Licensed Practical Nurse (LPN) provides direct clinical support in the delivery of preventive and primary care services within the HSVS 29-i Medical Clinic. Working under the supervision of the RN Supervisor and in collaboration with the Medical Director, Physician(s), and Nurse Practitioner(s), the LPN supports patient care, ensures accurate documentation, and assists in maintaining compliance with New York State Department of Health (NYSDOH) 29-i Health Facility requirements.

The LPN plays a key role in patient preparation, immunization support, clinical procedures, and ensuring the overall efficiency of clinic operations.

Active Licensed Practical Nurse (LPN) license in New York State required

Minimum 13 years of clinical experience, preferably in outpatient, pediatric, or adolescent care

Experience with Electronic Health Records

Knowledge of immunization practices and preventive care preferred
